General Description

University Health Services (UHS) at the Johns Hopkins School of Medicine is seeking a Nurse Practitioner to work collaboratively within a multidisciplinary health team to optimize team performance and patient outcomes and is responsible for providing comprehensive and age appropriate care to both well and/or ill patients. We are seeking an excellent clinician to provide clinical care to the health professions students, graduate students, house staff, and post-doctoral fellows at the Johns Hopkins East Baltimore Campus.

The Nurse Practitioner is responsible for providing high quality and effective patient care through the evaluation, diagnosis, and treatment of acute and chronic illnesses and injury. In addition to providing direct patient care, the Nurse Practitioner also educates patients with a significant emphasis on health maintenance and preventative measures. The Nurse Practitioner shares responsibility for the provision of excellent health care in compliance with state, federal and ambulatory accreditation requirements, and practices within the guidelines of University Health Services, Johns Hopkins Medicine and in accordance with the State of Maryland Board of Nursing. The Nurse Practitioner is granted affiliate staff privileges by the Johns Hopkins Hospital Medical Board; these privileges list specific tasks and are reviewed annually.

Primary Duties and Responsibilities:

Coordinate care for patients to achieve clinical and organizational outcomes. Uses advanced clinical and theoretical knowledge to identify acute and/or chronic health problems through a comprehensive, age appropriate assessment of patients using skills in taking medical histories, screening, physical examinations, ordering diagnostic tests, making tentative diagnoses, prescribing appropriate medications and treatments. Provides evidence-based care and provides patient education for self-care chronic disease management.

Coordinate medical consultations and arrange for medical follow-up at discharge for inpatients who have multiple active medical conditions and significant disease burden, under the supervision of the faculty attending physician.

Effectively uses resources. Prioritizes and organizes work to meet the changing priorities of UHS. Assesses lab results and communicates with patient in a timely and confidential manner, and implements intervention in consultation with primary providers and medical director. Provides guidance and direction to other staff where applicable to promote quality improvement/patient safety projects.

Works collaboratively with medical, psychiatric and surgical consultants. Strive to meets and/or exceed established productivity targets for services provided associated with UHS clinic visits.

Complete clinic notes within the UHS electronic medical records system on the day of visit and discharge summaries on the day of discharge for effective communication with primary care providers, medical consultants and informal care providers.

Accountable for own patient care decisions. On a regular and continuous basis, exercises administrative judgment and assumes responsibility for decisions, consequences, and results having an impact on people, costs, and/or quality of service within the functional area.

Required to participate in on-call after hours rotation shared by UHS providers, reachable by phone and/or pager. Required to participate in UHS interdisciplinary provider and staff meetings. Evening hours. Required to be flexible to accommodate evening clinic sessions.

Responsible for other clinical duties as assigned.

Qualifications

Master's degree in nursing; Registered Nurse license and Certified as a Nurse Practitioner. Must be licensed as NP in State of Maryland or other state where practicing. Three years of related CRNP experience strongly preferred. Demonstrated competence in the delivery of health care to a diverse group of young adult populations, specifically graduate or professional student populations and their families preferred. Current CPR certification required. Must maintain current licensure and certification during duration of employment. Completion of the JHH credentialing process is required prior to start date.

Additional Knowledge, Skills and Abilities:

_ Knowledge:_ Advanced clinical skills in specialty area or equivalent. Successfully completes all training courses required for access to Epic and other clinical software systems. General computer knowledge and capability to use computer. Requires effective use of resources and cost awareness.

Skills and Abilities: Ability to incorporate into practice theoretical concepts and advanced clinical skills with patients, families, and interdisciplinary health care team members. Ability to analyze data to synthesize and summarize information and to make judgments regarding patient care. Analytical ability necessary to diagnose patients and to develop and implement plans of action for long and short term management of acute and chronic health.Ability to apply principles of evidence-based practice, scientific process, and service excellence in practice.

Ability to interact successfully with the public. Ability to perform effectively despite sudden deadlines and changing priorities. Ability to maintain personal composure in high stress situations, deadlines and changing priorities.Demonstrated interpersonal, written and verbal communication skills are required to service a diverse customer base. Advanced interpersonal skills necessary to communicate effectively with all members of the interdisciplinary health care team to enhance collaboration and problem-solving.

Ability to communicate effectively, both verbally and in writing, in order to provide information to patients and health care personnel. Ability to demonstrate and convey a favorable image of the organization and to conform to proper standards of professional dress, attitude and demeanor. Able to operate patient care equipment to assess health; proficiency in using desktop computers or laptops, and general office equipment on a daily basis.

Physical Requirements: Work requires standing, bending, sitting and using/manipulating various health assessment instruments and machines, and/or walking for extended periods of time. Work may require positioning and assisting patients and transporting patients and/or equipment from one location to another with little likelihood for injury if proper body mechanics and procedures are followed.

Working Conditions: Possible exposure to communicable diseases, hazardous material, pharmacological agents, with little likelihood of harm if established health precautions are followed.
