The Nurse Home Visitor acts as a case manager and implements standard protocols for families enrolled in the Nurse Family Partnership.

Reports directly to the Nurse Family Partnership Nurse Supervisor.


*1. Provides overall coordination of care and home visitation for families enrolled in the Nurse-family Partnership.

*2. A full time Nurse Home Visitor carries a caseload of 25 clients or a proportionate number if part time or if caseload warrants (for example, if have clients on monthly visits may have caseload over 25 clients).

*3. Assesses physical, emotional, social and environmental needs of women and their families as they relate to the Nurse Family Partnership domains.

*4. Initiates a plan of visitation based on the standardized protocols of the Nurse-Family Partnership.

*5. Assists women and their families in establishing goals and outcomes.

*6. Provides anticipatory guidance and instruction to mothers and significant others regarding pregnancy, infants and toddlers based on the standardized protocols of the Nurse-Family Partnership.

*7.Provides education, support, and referral resources in assisting women and their families in attaining their targeted goals.

*8.Evaluates the effectiveness of the plan of visitation and the outcome of each visit.

*9. Adheres to the Nurse-Family Partnership model elements of home visitation.

*10. Completes and submits all appropriate documentation forms within 48 hours.

*11. Provides for discharge planning.

*12. Educates referral sources and general public about the Nurse-Family Partnership.

*13. Seeks referrals from referral sources. Develops and maintains community relationships to support client referrals.

*14. Completes all required Nurse-Family Partnership education/orientation.

*15. Develops therapeutic relationships, utilizes concepts of reflection and motivational interviewing with women and their families in a home visiting environment.

*16.Particiaptes in review and analysis of the ETO reports for achievements and areas for improvement.

*17. Complies with Agency Corporate Compliance Plan.

*18. Complies with HIPAA Privacy and Security Standards.

*19. Participates as a member of a committee and/or Continuous Quality Improvement (CQI) Task Force/Process Team as requested.

*20. Attends Agency meetings as assigned.

*21. Participates in the achievement of Agency goals.

*22. Communicates constructively in an honest and open manner.

*23. Demonstrates respect for immediate supervisor an co-workers.

*24. Available by assigned pager or other employee requested means during working hours.

*25. Performs other duties as assigned by immediate supervisor.



Demonstrates strong interpersonal, oral and written communication skills.

Demonstrates the ability to maintain accurate and legible progress notes.

Demonstrate the ability to comprehend, speak and write the English language

Must be able to work a flexible schedule including some evenings and weekends.


Possesses a license to practice as a registered nurse in the Commonwealth of Pennsylvania. Bachelor of Science in Nursing Preferred.

Two year recent experience in maternal-child health, public health, mental/behavioral nursing, pediatrics or home health nursing preferred.

Home visiting experience preferred.

Possesses a current Pennsylvania driver’s license and automobile insurance.


Must have access to independent transportation in order to comply with standards of job description.

Must be able to enter residence, hospital/clinic facility, physician office or its equivalent independently regardless of physical barriers.

Must be able to complete a visual and auditory assessment of clients.

Must be able to move and position a client as appropriate for the care required.
