Cooper University Hospital POPULATION CARE COORDINATOR in CAMDEN, New Jersey
Assists physicians and clinical steering committee in developing and implementing evidence based practices and care plans for target health population.
Conducts home, nursing, rehab, etc visits with patients.
Coordination of Medical Home activities for Horizon attributed members. These activities include, but are not limited to hospital discharge follow up, initial and ongoing assessments, care planning, coordination of services, communication between all care providers.
Tracking of patient information, including but not limited to care plan compliance, health screenings, quality metrics
Reporting of patient information to physicians, division, HHI.
Supervision and coordination of Horizon Medical Home support personnel, including but not limited to Medical Assistants, Social Workers and Health Coaches.
Scheduled Days / Hours: Monday - Friday; License (or Certification):
5 years or more.
Certified Case Mgmt (CCM) preferred.
Current NJ RN License.
Successful completion of the HHI/Duke/Rutgers Population Care Coordinator Training Program within 90 days of employment.
Experience in population or case management preferred.
