Provides pharmaceutical care through staffing and clinical rounds. Reviews and monitors physician orders for drug therapy. Serves as a drug information resource to nurses, physicians, and other healthcare workers. Collects statistical data and documents clinical interventions per procedure. Implements identified pharmacy initiatives as applicable to the pharmaceutical care center.
Responds to requests for drug information from health care providers and patients. Evaluates the usefulness of biomedical literature gathered and formulates responses to drug information requests based on analysis of the literature. Provides medication-use education and counsels patients and caregivers.
Builds the information base needed to design a medication therapy regimen. Determines the presence of any of medication problems in patient's current medication therapy.
Designs and provides effective in-service education for physicians, nurses, pharmacy technicians, and other practitioners on medication therapy issues.
Collects statistical data and creates recommends corrective action based on the results. Observes the MAR for accuracy and proper documentation and reconciles discrepancies. Utilizes and documents existing clinical intervention policies. Participates in Quality Assurance and Performance Improvement activities.
Manages the use of investigational drug products according to established protocols.
Prepares and dispenses medications using appropriate techniques, assuring completeness of the order.
Utilizes pharmacy support personnel effectively and checks the accuracy of the work of pharmacy support personnel. Prioritizes the workload and organizes the work flow, taking into account available resources.
Works primarily in the pharmacy and delivers medications to nursing units. Requires considerable walking, standing, pushing mobile cart of 50 pounds maximum and lifting 25 pounds of material. May be required to frequently stretch, bend, twist, squat, or kneel to perform job. Exposure to cancer chemotherapy agents. Otherwise, hazards are very minor such as minor cuts, bruises, and non-contaminated needle pricks. Exposed to various conditions throughout the workday such as walking on hard surfaces, climbing stairs and changes in temperature and humidity. Must be able to perform manipulative skills such as writing, typing, and data entry into the computer and automated dispensing machines. Must have use of both hands, be able to see clearly and read small type such as that on unit-dose medication packages and syringes. Must be able to speak and be understood by others. Must be able to hear normal sounds with moderate background noises.
Education, Experience and Certifications
Bachelor of Science Degree in Pharmacy required. Current North Carolina Pharmacy License or eligible for reciprocity required. Prior Hospital Pharmacy experience preferred.
