NHS is currently seeking Direct Care Workers to join our IDD Team in Clarion, PA.

Position summary or overview. Responsibilities include but are not limited to the following.

  • Providing individualized care to assist individuals with acquiring, maintaining, and improving on skills in the home and community settings.

  • Work is performed in accordance with appropriate procedures according to the individuals Person Centered Plan / Individual Support Plan (ISP) and also the supervision of the Program Specialist who reviews results through daily documentation notes, conversations with the individual, staff, and periodic meetings with the Supports Coordinator and other members of the treatment team.

  • While providing services, travel in the local community may be required (travel reimbursement of $ .40/mi will be accrued when providing 1 to 1 services with the individual). Meal and or activity reimbursement will be provided during services if it relates to an outcome listed in the ISP.
