Duties include: (1) scheduling and completion of functional assessments within the assigned service region in the most cost-effective and efficient manner possible, (2) review of internal documentation to ensure consistency between data gathered during functional assessments and member-specific information contained in internal records; (3) entry of functional assessment data using an internet-based program that calculates a level of care; and (4) timely submission of accurate and appropriate paperwork. The Certified Screen Specialist transmits internal updates as warranted to ensure appropriate updates to acuity-based provider payment rates and appropriate reimbursement to Community Link Inc via the per member per month capitation payment from DHS. When warranted, refers members to other internal staff for individualized assistance that ensures member healthy and safety, or continuity of care for members who experience a change that impacts level of care or ongoing eligibility for benefits.PRINCIPAL DUTIES AND RESPONSIBILITIES* Upon assignment, schedules appointments with members to complete an annual redetermination of functional eligibility, or a redetermination of eligibility based on changes in functional abilities reported by Member Support Coordination (MSC) staff. Screens are expected to be scheduled in a manner which maximizes organizational resources, including time/travel expenses.* Uses the screening tool designed by DHS to gather information about each individual's functional abilities, health status, and personal and professional supports required to meet his or her desired level of functioning.* Pending the revelation of needs or concerns beyond the scope of the functional screen, refers members to staff of CCCW or an Aging and Disability Resource Center.* Responds to urgent situations and refers individuals for crisis intervention, as necessary.* Maintains up-to-date online member information regarding functional abilities, demographics, and appropriate contacts.* Works directly with MSC staff to ensure consistency between the information gathered during a member's functional assessment and member-specific data contained in internal records.* Conducts face-to-face interviews with individuals, usually in their homes, to perform the functional screen. Must have at least one year of experience working with the populations served by Community Link Inc (frail older adults, or adults with physical or developmental disabilities). Must successfully complete online certification to complete functional eligibility screens within 30 days of hire. Social work or nursing case management experience with older adults or adults with disabilities preferred. Knowledge of physical, psycho-social, and economic needs of individuals served by Community Link Inc. Ability to assess the functional needs of members. Familiarity with medical terminology, medical diagnoses, and treatment options. Ability to generate timely and accurate service records. Personal qualities needed include good listening skills, sen itivity, patience, ability to establish trust and rapport, creativity, and initiative. Ability to use word processor and spreadsheet programs on a personal computer to produce reports/correspondence.
