Southern Tier Aids Program Senior Care Manager (Ithaca Office) in Ithaca, New York
The Southern Tier Care Coordination (Southern Tier Aids Program) is seeking a Senior Care Manager in their Ithaca office to provide supervision for Care Managers within designated regional offices in their work providing confidential outreach, intake and care management services to persons with qualifying chronic illnesses. These include including People Living With HIV/AIDS (PLWH/A) and qualifying members of the Medicaid Health Homes care management program.
Qualifications: Must have a Bachelor's degree in Community Mental Health, Social Work, Nursing, Psychology, Sociology; Must have a minimum of two years' experience providing direct services to persons with a history of mental illness and/or chemical dependency. Additional experience working with the following populations also desired: persons with chronic illnesses and homelessness. Previous supervisory experience required. Knowledge of the Medicaid Health Homes program preferred.
Excellent organizational, time management and communication skills are essential. Computer literacy required.
Salary: $32,000 annually working 35-hours per week. Excellent benefits package including: medical, dental insurance, flexible spending plan, life insurance, 401(k) retirement plan, and generous time off (24 days annually and 11 paid holidays).
