Clinical Liaison - 161597Lakewood, CO 80235Position Summary: The Clinical Liaison provides clinical education andmarketing services to hospital and physician referral sources to ensureawareness of Preferred Homecare s programs and services. Assesses andcoordinates home infusion nursing services with appropriate candidates toassist with determining the plan of care administered by Preferred Homecareunder the direction of the patient s physician. Consults with physicians,pharmacists and other health care personnel to determine the best clinicalcourse of treatment for the patient.Essential Duties and Responsibilities include the following. Other duties maybe assigned. - Maintains and grows core therapy admits through customer relationshipsand provides responsive customer service with assigned accounts. Evaluatescurrent and future clinical and service needs for existing and new accounts. - Provide patient and caregiver training on therapy, monitoring, andside effects. Demonstrate procedure for administering therapy in the home,with a return demonstration from the patient/caregiver. - Demonstrates thorough knowledge of core therapy referrals,qualifying criteria both by payor source and branch. - Establish and maintain clinical documentation required to trackpatient teaching provided and any other clinical documentation requested byPreferred Homecare - Collaborates with Case Managers, Payors, referral sources, homehealth agencies and Preferred Homecare staff to facilitate patient dischargeand coordination of care. - Recognizes opportunities for expanding the business relationships withexisting accounts and involves the Territory Manager, Regional Director ofSales, or the VP of Sales when appropriate to present Preferred Homecare sother programs or negotiate terms of service. - Provides clinical expertise in support of the sales effort byutilizing clinical skills to assist referral sources with identification ofappropriate home infusion candidates, provides assessment of patientsability to perform care independently or with the assistance of an appropriatecaregiver, provides patient teaching regarding therapy, supplies, andsafety. Provide direct patient care through hospital hook-ups, enteral pumpteaches or other patient care needs in order to facilitate discharge from afac - Appropriately documents patient care activity, coordination of homeinfusion nursing services provided and all communication with patients,physician and referral source. - Ensures all documentation is legible, accurate, complete, andcomplies with company policy and procedure for electronic and paper patientclinical record entries. - Communicates/coordinates discharge planning activities for newpatient referrals with branch personnel (Admissions, Clinical,Operations, and Reimbursement and Referral sources). - Answers incoming telephone calls and triages to appropriate person asindicated. - Participates in staff meetings with nursing and pharmacy. - Participates in departmental QA activities as requested. - Participates in cross training of the various clinical functions inorder to function in multiple roles as required. - Attends designated departmental and branch meetings as scheduled.Education/Experience: - Bachelor's degree (B.
