Summary of Job Function: Assumes responsibility for direct supervision of the case management Department. Coordinates the care of the patients across the continuum of care. Responsible for addressing utilization management, discharge planning activities in order to optimize patient outcomes within appropriate time frames. Facilitates the proper use of resources. Assists in the development and coordination of the Case Management program. Maintains compliance with regulatory standards within the scope of Case Management. Responsibilities include: Outcomes Management, Utilization Management, Reporting/Data Analysis and Staff Management. Is responsible for reporting identified safety issues such as hazardous environments (i.e. damaged floors/walls/ceiling tiles/unsecured areas) and medical errors. Insure that [unit/department/division] is in compliance with all applicable policies, laws and regulations. Job Qualifications: Currently licensed Registered Nurse with a BSN from an accredited college or equivalent. (Master s degree preferred). Minimum of five years clinical experience. Minimum of three years experience in Case Management. Minimum of one year supervisory experience. Successful completion of all Orientation Programs. InterQual training required annually. For new hires, InterQual training is required within 30 days of hire date and annually thereafter. Must be able to type 30 WPM and demonstrate knowledge of computers, word processing, Excel, email fax, and telephone skills.
