The Sarah Chudnow Community's Memory Care program features a secure family-like environment that includes individual resident rooms, a communal kitchen, and dining and leisure areas for nine residents. Job Summary : Provides prescribed medical treatment, medication and personal care services to nursing home residents to meet individual spiritual, physical, emotional and social needs consistent with the plan of care, standard nursing practice, scope of gerontological nursing procedures and all applicable regulations, codes, policies and procedures. We currently have a part-time 0.3 position available on 2rd shift. Oversees, directs, reviews and evaluates work performance of nursing staff and participates in quality assurance or educational activities on assigned shift to determine and ensure conformance with current facility State and Federal regulations. Administers and documents medications, treatments, etc. within the parameters of the RN licensure and according to the care plan. Assists with obtaining, transcribing and follow through of physician's orders. Orders, properly stores, administers and records medications and treatment supplies and notes their results. Assists with acquisition, maintenance, and proper use of supplies and equipment, etc. to ensure adequate levels, which conform to care plan and meet individual needs of the resident. Writes timely nursing summaries that reflect residents' conditions, cares being given, restorative/rehabilitation interventionism and the movement toward appropriate goals and acceptable approaches. Makes rounds with medical director and attending physicians. Communicates pertinent information to staff on assigned shift and to the following shift as necessary. Periodically audits residents' charts, flow sheets, and other documentation for accuracy and completeness; reviews and revises care plans as changes occur so as to reflect the exact condition of the resident. Graduate of an accredited school of nursing with a Registered Nursing degree. Prior nursing home experience helpful, but not necessary. Sufficient education to demonstrate spoken and written English. Demonstrates strong communication and relationship building skills with residents, and is able to explain residents' needs to others as needed for resident advocacy.
