Position Summary: The Home Health Team RN Leader is a professional registered nurse who functions as the liaison between Department day to day nursing responsibilities and the Clinical Manager. The RN Team Leader is responsible for daily support, guidance, education, and field supervision of patient care conducted by the nursing team. The Home Health RN Team Leader serves as the administrative representative on evenings, nights, weekends and holidays. MINIMUM QUALIFICATIONS: Education: BSN or Bachelor's in related health care field, Master's preferred Required Length and Type of Experience: Three (3) years of clinical experience in a home health or hospice setting. Two (2) years in leadership or educator role preferred. Required Licensure, Certification or Registry: Current license by the Ohio State Board of Nursing. CPR certification required. OASIS certification required, or obtained within one year of hire. PRINCIPAL DUTIES AND RESPONSIBILITIES: · Primary responsibility is to provide daily support as the team leader within the nursing staff. Coordinates scheduling, both patient and nursing staff, completes regular supervisory visits, provides field support and education, responsible for co-visits on complicated patients, and functions as a liaison between agency nursing roles and the Clinical Manager. Facilitates nursing staff with completion of patient care, competency with related procedures, obtaining ordered laboratory tests and supplies. Interprets and reinforces Health Center policies and values to associates. · Responsible for leading all forms of education to improve clinical care. Provides expert Oasis guidance. Promotes associate growth by providing learning opportunities, guidance, counseling and support. Serves as a designated mentor to new Home Health nursing staff. · Responsible for cost containment in the department. Performs administrative duties related to nursing care within the agency, and acts as a role model for the Home Health Agency Nursing staff (RNs, LPNs, Home Health Aides). Monitors daily patient scheduling, nurse continuity, Visionware labor, and administrative nursing reports. Recommends appropriate staffing based on objective information, i.e., Staff mix, census, acuity. Provides input to support budget decisions for clinical staffing. · Participates in quality process improvement (PI) initiatives. Provides suggestions for opportunities for improvement. Individually or with team, provides documented and measurable improvement in quality, service and satisfaction standard. Participates in a leadership role (chairperson/project manager) in Dept PI. · Assists with throughput issues from referral to agency discharge. Works in collaboration with referral sources and agency nursing to facilitate admission/transfers/discharges. Ability to communicate appropriately the needs of throughput to staff. Acts as a liaison between Health Center Departments, physicians, and referral sources. Supports interdepartmental working relationships and facilitates problem solving. Maintains open communication to all Health Center departments to promote effective customer service relationships. Addresses intra/interdepartmental issues in an open, constructive manner. · Supports working relationship and offers to assist others when work is complete without being prompted. Addresses coworker issues directly in adult-adult manner in private. Takes responsibility to initiate and participate in the resolution of conflict with or between co-workers. Supports leadership. Adapts to changes and priorities in work activities and structure. Does not complain without solutions. · Key player on patient satisfaction initiatives and role models behavior. Follows and role models Health Center policies, procedures, practices and standards. · Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards.
