BAYADA Home Health Care is seeking a Hospice Clinical Nurse Manager to lead care teams committed to delivering the finest hospice services in the country. As a Hospice Clinical Nurse Manager, you will share your vision by leading a hospice interdisciplinary team and ensuring excellent clinical and social care. You will develop relationships within the community and local facilities, collaborate with other hospice providers, recruit core hospice team members, manage cases and field staff, and participate in fieldwork as needed.

As a Nurse Manager, you will:

  • Demonstrate and communicate the core values of BAYADA Hospice and The BAYADA Way.

  • Conduct client assessments and complete the care planning process.

  • Perform a comprehensive client assessment at admission and on an ongoing basis, initiating referrals for additional services as needed to obtain the highest quality outcomes for client.

  • Establish and update the care plan and goals, in collaboration with the IDG, client and family.

  • Consult with the physician and obtain physician’s orders for care as required and per policy.

  • Support the team by providing clinical guidance and judgment to field and office colleagues to promote understanding of and response to client’s needs, participating in Quality Improvement activities, and assisting in developing Clinical Associates to become Clinical Managers.

As a member of the Interdisciplinary Group (IDG), you will be responsible for clinical and client case management, acting as the internal case manager and client advocate. You will communicate and collaborate with IDG members regarding client status, care plan and expected outcomes, making regular client visits, as per policy, to evaluate client progress toward goals. This unique position allows you to keep your clinical skills strong, while at the same time developing relationships and leadership skills.

Qualifications for a Nurse Manager:

  • Current Vermont RN license

  • Graduate of an accredited and approved nursing program as indicated by school transcript or diploma, BSN preferred, and required based on state regulations

  • Strong hospice case management experience required

  • Prior supervisory experience required

  • Strong knowledge of hospice regulations

  • Passion for focusing on hospice care

  • Ability to function independently with minimal supervision

  • Desire to lead an interdisciplinary care team

  • Ability to travel as needed
