NHS is currently seeking a Clinical Assessor to join our Adult Behavioral Health Division in our Shreveport, LA location.

Position summary or overview. Responsibilities include but are not limited to the following.

The Clinical Assessor will travel to various sites to conduct assessments with new and existing clients. Depending on which type of assessment is required, the employee will be responsible to determine whether the medical necessity exists for either ACT services or nursing home placement. For the follow up hospitalization assessments, the employee will be responsible for ensuring that the consumer has an appropriate and attainable discharge plan in place.

  1. Keep open communication with Coordinators and/or Administrative staff on referrals for potential and existing adult clients.

  2. Complete individual assessments by traveling throughout the community to meet with clients, their families, and/or others involved in their treatment and services.

  3. Determine, based on the outcome of the completed assessment, whether existing clients meet the criteria for continued placement or services and for new clients determine whether placement or services is medically necessary.

  4. Make recommendations as to what provisions the facilities should have in place to meet the clients' needs.

  5. Work a flexible schedule at times as per business necessity.

  6. Work independently and with minimal supervision.

  7. Maintain complete, timely and appropriate documentation in per program requirements. Complete reports and correspondence as needed.

  8. Provide consultation to NHS staff with regard to client care and treatment.

  9. Adhere to all NHS, County, State, and Federal policies and regulations related to mental health and addiction treatment services.

  10. Complies with NHS ethics and code of conduct, as well as the ethics and code of conduct of own discipline, in contact with all clients, co-workers, and the community.

  11. Participates in regular clinical supervision and continuing education activities.

  12. Other duties as assigned.

The Clinical Assessor must be (LPC or LCSW) and will complete PASRR Assessments, Follow up Hospitalization Assessments and Behavioral Health Assessments within the community. This position requires travel and potential for weekends.
