Under the direct supervision of the RN Case Manager, provides continuous care services, in-home respite care services, and personal care for the patient and assists with activities of daily living. Supports the Provider Relations activities of the organization.

Essential Job Functions

  • Deliversnursing services in accordance with hospice care policies and acceptedstandards of practice under the direction of the registered nurse. Follows the plan of care. Is alert to the patient’s needs and concernsand teaches appropriate self-care techniques to patients and family members. Administers medications

  • Providespersonal care, including: bathing,dressing, grooming, caring for hair, nail care, oral hygiene, skin care, andfoot care

  • Preparessupplies and equipment for treatments that require sterile or aseptic techniquein accordance with the Plan of Care

  • Assistspatients in turning, getting up in a chair, or ambulation according to the planof care. Assists with elimination,including: bedpan, urinal, up to thebathroom, routine Foley catheter care and enemas according to the plan ofcare. Assists patients with meals, ifnecessary

  • Performshousehold services essential to maintaining a safe and comfortable environment

  • Assiststhe attending physician or registered nurse in performing specializedprocedures as an extension of therapeutic services, prepares equipment,supplies, and treatments that require using sterile or aseptic technique, inaccordance with the patient’s plan of care

  • Observespatients when giving care and reports changes in the patient’s condition andneeds to the director of clinical services and/or patient care coordinator

  • Completesappropriate clinical progress notes and documentation in a timely manner

  • Demonstratesa compassionate attitude toward patients and families

  • Attendspatient care conferences as requested by the director of clinical servicesand/or patient care coordinator. Contributes to team effort by accomplishing related results as needed.From time to time this position will be expected to perform on-callprofessional duties


  • Must hold and maintain a current license to practice as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in practicing state

  • Minimum two (2) years nursing experience

  • Minimum one (1) year hospice experience, preferred

  • Current CPR certification

  • Must have valid driver's license and 24-hour access to a motor vehicle and maintain personal auto liability insurance coverage

  • Must be capable of maintaining regular attendance

  • Must possess computer skills of the types and at a level necessary to accomplish the job

  • Must be able to maintain confidentiality regarding patient and company proprietary information

  • Must have the ability to relate professionally and positively and work cooperatively with patients, families, and other employees at all levels
