Position Purpose:

Assists in planning, developing, and coordinating appropriate social, physical, mental and educational activities to enhance and promote the therapeutic goals for adult day health participants. Responsible for coordinating and conducting group and individual activities. Maintains a clean and safe environment and assists with personal care.

Job Description Details:

PRIMARY JOB RESPONSIBILITIES AND DUTIES:

Demonstrates, promotes and integrates the Sisters of Providence Health System’s mission and core values by:

  • Assists in planning the monthly activity calendar.

  • Implements planned activities daily.

  • Helps to transport participants to and from activities. Capable of pushing participants in wheel chairs

  • Works as part of the interdisciplinary team to promote holistic health care for each participant

  • Assist with the care of the facility’s pets.

  • Reports significant attitudes and behaviors of participants during activities to the charge nurse or nursing supervisor.

  • Observes participants and reports any reactions or changes in condition to appropriate personnel.

  • Assists in caring for and maintaining program space to ensure a safe, clean environment. (Example: Wash dishes, clean tables, vacuum floors, clean bathrooms, dust, take out trash, etc.)

  • Provides personal care of participants by assisting with ADL’s as needed. Personal care may include shower/ bath, shampoo, transfers, positioning, nail care, ambulation, toileting, and ensuring that adaptive devices are worn.

  • Escorts participants to and from the dining room for meals and snacks, feeds participants as assigned, records intake of solids and fluids and supervise those participants who need assistance. Distributes nourishments to participants as assigned and accepts responsibility for reporting to nursing if refused by the participant.

  • Follows all safety, security, infection control and hazardous materials policies and procedures. Performs all tasks to ensure participant and personal safety and the protection of co-workers and facility property.

  • Promotes and participates in educational programs. Annually attends all required educational programs according to administrative policies and procedures. Maintains required certification.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:

  • High School diploma or its equivalent. Certified Nursing Assistant preferred.

  • Prior experience working with the older population and knowledge of Therapeutic Recreation or activities.

  • Must be able to read, write and converse in English. A second language is helpful.

  • Interpersonal skills necessary to relate to persons of varying age, culture, religion, mental status, and/or ability

  • Requires light physical effort on a regular basis, as in frequent standing, walking, bending, stooping or reaching and/or occasional lifting of light items. (up to 20 lbs.) Pushing/pulling wheelchairs. In emergency situations may be required to assist in lifting a person who has fallen,

WORKING CONDITIONS:

  • Physical surroundings are pleasant and comfortable, with occasional exposure to injury or other health hazards.

  • Must be able to communicate orally and in writing.

  • Little or no apparent stress or tension in work dealing with associates.

REPORTING RELATIONSHIPS:

  • The Program Assistant reports directly to the Program Director.
